PAMI stands for Primary Angioplasty in Myocardial Infarction. It is a specialized, time-sensitive emergency cardiac intervention performed immediately after a patient suffers an acute heart attack. In a heart attack, a ruptured cholesterol plaque or blood clot suddenly cuts off the blood flow inside a major coronary artery. PAMI involves performing catheterization right away to break the clot, dilate the artery, and place a stent, restoring vital blood flow.
During a heart attack, every second of delay leads to irreversible death of myocardial (heart muscle) cells. Medical guidelines strongly mandate a **"door-to-balloon" time of under 90 minutes**—the span between when a patient enters the hospital doors to when the coronary balloon is inflated. The emergency procedure requires no general anesthesia or open-heart surgery, operating entirely through a micro-incision:
A very thin, flexible tube is inserted via the wrist (radial artery) or groin (femoral artery) and guided directly to the heart.
A tiny balloon at the catheter tip is inflated exactly at the block site to compress the clot and open the closed artery path.
A drug-eluting stent (mesh tube) is permanently expanded to act as scaffolding, holding the artery open to ensure blood flow.
